When you contact us one of our volunteers will take your details and will provide you with initial advice and, if appropriate, book an office or home appointment for you.
If you telephone us when the office is closed (or if our lines are busy when you call) please leave a message including your contact number and we will call you back as soon as we can.

We are on the top floor of Bedford House. There is ramp access to the main door and lift access to the top floor.
However, if you would like to send us your details rather than telephoning us please click here to download our Information Request form which we would like you to print and complete.
If you are providing us with details about yourself you only need to complete the first page. You can ignore the second page.
However, if you are providing us with details about someone else ie. a client or someone you care for please provide details about him or her on the first page and about yourself on the second page.
One of the boxes on the first page asks for details of ethnic background. You are asked to select a code which you will find as a list on page three of the Information Request form.
When you have filled in the form(s) please send the documents to
us by post.
Once we have received your details we will normally telephone you to offer you an appointment or to offer you advice.
